【摘要】 目的 探討患者胃黏膜是否有內(nèi)皮素1(ET1)、血管內(nèi)皮生長(zhǎng)因子(VEGF)和缺氧誘導(dǎo)因子1(HIF1)在門脈高壓性胃病(PHG)血流動(dòng)力學(xué)調(diào)節(jié)中的意義。方法 分別采取PHG、PHT無(wú)PHG、正常對(duì)照患者胃黏膜標(biāo)本用免疫組織化學(xué)方法分別檢測(cè)ET1、VEGF和HIF1在胃黏膜中的表達(dá)。同時(shí)測(cè)定3組患者的門靜脈血流量(portal venous flow,PVF)和脾靜脈血流量(spleen venous flow,SVF)。結(jié)果 PHG積分與食管靜脈曲張程度無(wú)相關(guān)性(HC=4.782,P>0.05)。PHG組PVF和SVF水平明顯高于正常對(duì)照組(P<0.05)。PHG組胃黏膜中ET1、VEGF、HIF1的陽(yáng)性面積高于正常對(duì)照組(P<0.05)。ET1、VEGF、HIF與PHG積分呈正相關(guān)(H分別為10.592、10.614、9.852,P<0.05)。PHG組患者HIF1陽(yáng)性表達(dá)和VEGF的陽(yáng)性表達(dá)之呈直線相關(guān)(Pearson相關(guān)數(shù)為0.842,P<0.01)。結(jié)論 ET1、VEGF和HIF1與PHG的發(fā)生發(fā)展有相關(guān)性。

Endothelin1,vascular endothelial growth factor and hypoxia inducible factor1 in patients with portal hypertensive gastropathy LI Shujuan,SHAO Shixiang,GENG Guoyu,et al.The Fifth Hospital of Shijiazhuang, Shijiazhuang 050021,China

【Abstract】 Objective To investigate the significance of endothelin1(ET1),vascular endothelial growth factor (VEGF) and hypoxiainducible factor 1 (HIF1)in patients with portal hypertensive gastropathy(PHG).Methods The specimens of gastric mucosa were divided into three groups,patients with PHG group, PHT without PHG group and control group.The specimens were detected by immunohistochemical methods for the levels of ET1,VEGF and HIF1 respectvely.At the same time,the portal vein blood flow(PVF)and splenic blood flow(SVF)were detected in the three groups.Results PHG scoring was not related with the degree of esophageal varices(P>0.05). The levels of PVF and SVF in PHG group were significantly higher than those of control group(P<0.05). The positive area of ET1, VEGF and HIF1in PHG group was significantly higher than that of control group (P<0.05). The levels of ET1, VEGF,HIF1 were positively correlated with PHG scoring(P<0.05). The positive expression of HIF1in PHG group showed a straight line correlation with the positive expression of VEGF (Pearson correlation coefficient:0.842,P<0.01).Conclusion The levels of ET1,VEGF and HIF1 are correlated with the pathogenesis and development of PHG.
